In consideration of my child’s participation in the programs, classes, events, and use of the facilities, equipment, and services provided by MukTown Tumble LLC (“MukTown Tumble”), I, the undersigned parent or legal guardian, agree as follows:

1. Acknowledgment of Risks

I understand that gymnastics and tumbling involve strenuous physical activity, including but not limited to twisting, tumbling, jumping, climbing, rotation, and the use of equipment/apparatus. I acknowledge that participation may result in serious injury or death, including but not limited to: sprains, strains, fractures, dislocations, concussions, spinal cord injury, paralysis, or other severe conditions. I affirm that it is my sole responsibility to determine my child’s physical and mental readiness for participation.

2. Release of Liability I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby release and discharge MukTown Tumble LLC, its owners, officers, employees, instructors, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to my child’s participation, whether caused by negligence or otherwise.

3. Indemnification, I agree to Indemnify, defend and hold harmless MukTown Tumble and its representatives from any loss, liability, damage, or expense incurred in connection with my child’s participation, including claims brought by third parties.

4. Assumption of Responsibility I voluntarily assume full responsibility for any risk of bodily injury, death, or property damage to my child or myself resulting from participation, use of facilities, or equipment at MukTown Tumble.

5. Medical Authorization-In the event of injury or emergency, I authorize MukTown Tumble to seek medical treatment for my child. I agree that I am solely responsible for any and all medical expenses resulting from such treatment.

6. Media Release-I authorize MukTown Tumble to use photographs, videos, or likenesses of my child for promotional purposes, including but not limited to social media, advertising, and its website. I understand that no compensation will be provided.

7. Severability & Governing Law-This agreement shall be governed by the laws of the State of Wisconsin. If any provision is held invalid, the remainder shall remain in full legal effect.

8. Acknowledgment of Understanding-I certify that I have carefully read this waiver, fully understand its contents, and voluntarily sign it. I acknowledge that by Checking the box to this agreement, I am waiving certain legal rights, including the right to sue.
